Dr. Sakamoto is one of the top dentists in Tokyo with over 30 years of experience.

“In Japan, we follow a completely different philosophy,” he said.

"Western dental care focuses on protection and preventing damage. We focus on regeneration — actually repairing the damage that happens."

He explains: "Your teeth are constantly under attack. Acids, bacteria, even the foods you eat create small damage every single day."

“Fluoride, used in almost all western toothpaste, creates a protective layer called 'fluorapatite' that helps resist acid."

“But in Japan, we use a different ingredient called 'nano-hydroxyapatite'.

A study from 1983 to 1986 compared hydroxyapatite to fluoride in Japanese schoolchildren who brushed daily for three years.

He pointed to the bars. The difference was significant.

We asked: "What's so different about nano-hydroxyapatite?"

Dr. Sakamoto explained: "Nano-hydroxyapatite particles don’t just sit on the surface like fluoride...

They're chemically identical to your teeth — It’s the same natural mineral that makes up 97% of your teeth.

They connect with your existing tooth structure, fill damaged spots, and become part of your existing tooth enamel again."

That means less fillings, less root canals, less crowns, and less money spent on dental visits.

I asked him why this wasn't common knowledge in the US.

He paused. “I can’t speak for US dental associations. But here in Japan we’ve had decades of research on this and have been using it ever since.”

After my conversation with Dr. Sakamoto, I wanted to try nano-hydroxyapatite toothpaste myself.

So I started researching American brands. That's when I stumbled onto something alarming.

A recent lab test looked at 37 “hydroxyapatite” toothpastes sold in the US.

The results?

They’ve identified four things that separate repair from marketing hype:

1. Particle Size: Only nano-sized particles are small enough to bond with your teeth. Many brands use micro-hydroxyapatite, which is too large to do anything meaningful.

The top-rated product, with a score of 97/100, was "Repair+ Remineralizing Toothpaste" by Rethinkteeth.

The lab results showed why it stood out:
